Community Health Champions โœจ cross-curricular

Teacher: TeacherAI | Grade: 5 | Subject: Reading/ELA, Health Education, Social-Emotional Learning | Duration: 60 minutes

๐Ÿ“ Description: Students analyze how community systems affect health habits and create action plans to become health champions in their communities.

Standards

  • 5.L.6 (Acquire and use accurately grade-appropriate general academic and domain-specific words and phrases)
  • HE.5.7.3 (Explain the importance of making health and safety practices into health habits)
  • SEL.5.SOC.5 (Understand how systems and structures affect community well-being)
  • SEL.5.RDM.5 (Reflect on decisions, take responsibility, and apply learning to future situations)

Learning Objectives

Students will be able to:

  • Identify three specific community systems that influence health habits (parks, grocery stores, schools)
  • Analyze how positive and negative community factors affect individual health decisions
  • Create a personal action plan connecting daily health habits to community leadership
  • Reflect on past health decisions and explain how they will apply learning to future choices

Supplies Needed

  • Tablets or Chromebooks
  • Chart paper
  • Fine-tip markers
  • Research notebook

Lesson Structure

Opening (5 minutes)

Display the question: "What makes it easy or hard for people in our community to be healthy?" Have students think-pair-share for 2 minutes. Call on 3-4 pairs to share one idea each, recording responses on whiteboard under "Easy" and "Hard" columns.

Main Activity (50 minutes)

Step-by-step instructions:

  1. Community Systems Introduction (8 minutes): Introduce the term "community systems" and explain three key systems: built environment (parks, sidewalks), food environment (stores, restaurants), and social environment (schools, community centers). Have students add vocabulary terms to research notebooks with definitions.
  2. Digital Research Phase (15 minutes): In pairs, students use tablets to research one assigned community system. Provide guiding questions: "How does this system help or hurt healthy habits?" "What examples can you find?" Students record findings using the Community Systems Analysis sheet in their research notebooks.
  3. System Analysis Gallery Walk (10 minutes): Each pair creates a mini-poster on chart paper showing their system, one positive impact, one negative impact, and a real example from research. Post around room and conduct gallery walk where pairs rotate to view and add sticky note questions or connections.
  4. Personal Reflection Mapping (8 minutes): Students complete the "My Health Habits Reflection" worksheet, identifying one current health habit, how community systems help or hinder it, and one past decision they want to improve.
  5. Health Champion Action Planning (9 minutes): Using their reflection, students create a 3-step personal action plan: 1) One habit to strengthen, 2) One community resource to use, 3) One way they can help others. Encourage specific, measurable goals like "Walk to the park twice this week" rather than "Exercise more."

Closing (5 minutes)

Have students share their #3 action item (helping others) with a partner. Ask for 2-3 volunteers to share how they plan to be health champions. Emphasize that small daily choices build into lifelong habits and community leadership.

Quick Check: "Name one community system that affects health. How can daily habits build leadership skills? What's one thing you'll do differently this week?"

Formative Assessment

During the lesson, look for:

  • Students connecting specific community features to health outcomes during research discussions
  • Use of vocabulary terms (community systems, built environment, habits) in verbal and written responses
  • Reflection quality - students making connections between past decisions and future planning

Differentiation Strategies

Support for Struggling Students:

  • Provide sentence starters for research recording: "This system helps health by..." "One example is..."
  • Partner struggling readers with strong readers for research phase
  • Offer choice of drawing or writing for action plan creation

Challenge for Advanced Learners:

  • Research multiple community examples and compare different neighborhoods or cities
  • Create proposals for community improvements with specific implementation steps
  • Interview family members about health habit changes over time and community influence

ELL/ELD Support:

  • Pre-teach key vocabulary with visual examples (show pictures of parks, grocery stores, etc.)
  • Provide bilingual research options and encourage native language discussion before English sharing
  • Use graphic organizers with pictures and sentence frames for action planning
